Before You Book

Do I need a referral to schedule a consultation?

No. You can request a consultation directly. No referral is needed from another doctor or specialist. If you were referred by a healthcare provider, please mention that in the form so your care coordinator can coordinate with them.

What happens after I submit the form?

A care coordinator will contact you within one business day. They will discuss your health concerns, answer questions about the process, and help you select the right appointment type. They can also walk you through intake forms and what to bring.

Can I book an appointment online without calling?

The form on this page is a consultation request, not a self-service booking tool. Because Progressive Medical Center provides highly individualized care, a care coordinator reviews each request to match you with the right provider and appointment type. This ensures your first visit is productive from the start.

How much does a consultation cost?

Your care coordinator will walk through pricing with you during your call. Progressive Medical Center is out-of-network for all insurance providers. Superbills are available so you can submit to your insurer for potential reimbursement. Cherry Financing and CareCredit are also available. Visit our Insurance and Financing page for full details.

I am traveling from out of town. Can I still request an appointment?

Yes. Many patients travel from across the country and around the world. When you submit the form, mention that you are traveling so the care coordinator can connect you with the patient navigation team. What should I have ready before my first visit?

Your care coordinator will guide you through everything, but it helps to have your completed intake forms, a list of current medications, any recent lab results, and questions for your provider. Our New Patient Information page has a complete checklist.
